Grading system

Different approaches are used to grade gynecomastia. The most common one is Simon’s grading which goes to the year 1973. According to this grade, there are 3 different categories as the loose skin and breast tissue amount on the breast.

•      Grade I

Small enlargement but no presence of excess skin

•      Grade IIa

Moderate enlargement but excess skin is not there

•      Grade IIb

Moderate enlargement with minor skin excess

•      Grade III

Marked enlargement with excess skin in greater amounts. Moreover, it gives the appearance of a female-like breast.

One of the other systems is Rohrich. It was developed in 2003 and the grading in done on 4 basis:

•      Grade I

Minimal hypertrophy (< 250 g) without ptosis

•      Grade II

Moderate hypertrophy (250 to 500 g) without ptosis

•      Grade III

Severe hypertrophy (> 500 g) with grade I ptosis

•      Grade IV

Severe hypertrophy with grade II or grade III ptosis

Both of them are used all across the globe. Simon's grading is more commonly used as it is fairly easy to understand. Along with the grading system, there is the skin tone factor. The excess skin is referred to as the loose skin which is present on the top of breast tissue. The excess skin is referred to as linear progression from 1 to 3 grades and skin tone is not dependent on it. Here is the revised grading for your understanding.
