Grading system
Different approaches are used to grade gynecomastia. The most common one is Simon’s grading which goes to the year 1973. According to this grade, there are 3 different categories as the loose skin and breast tissue amount on the breast.
• Grade I
Small enlargement but no presence of excess skin
• Grade IIa
Moderate enlargement but excess skin is not there
• Grade IIb
Moderate enlargement with minor skin excess
• Grade III

One of the other systems is Rohrich. It was developed in 2003 and the grading in done on 4 basis:

Minimal hypertrophy (< 250 g) without ptosis

Severe hypertrophy (> 500 g) with grade I ptosis
• Grade IV
Severe hypertrophy with grade II or grade III ptosis
Both of them are used all across the globe. Simon's grading is more commonly used as it is fairly easy to understand. Along with the grading system, there is the skin tone factor. The excess skin is referred to as the loose skin which is present on the top of breast tissue.
